KilgourMD is a well-formulated, drug-free scalp system with a better trial than most, and a price that only makes sense if minoxidil is off the table for you. Its own study found less shedding over four months, but it had no placebo group, so it cannot show how much of that the serums caused.
Below is what is in each product, what the trial measured, what it costs per month, and the readers who should try it and the ones who should not.

From the editor
Readers ask about KilgourMD because it is the first "dermatologist-founded" serum many of them have seen with a published study attached. That is worth taking seriously.
It is also worth reading what kind of study it is. An open-label trial tells you people improved while using it, not that the product made the difference.

The short verdict
Try it if you have early female-pattern thinning or increased shedding, you do not want minoxidil, and about $150 a month is affordable for four months. The serums use the best-studied cosmetic actives in one routine.
Skip it if your thinning is progressing, you want the strongest evidence for the money, or your scalp reacts to fragrance; every product contains it. Minoxidil has decades of controlled trials and costs a fraction of the price.
What KilgourMD is
KilgourMD was launched in 2023 by Dr. James Kilgour, who the brand describes as a board-certified dermatologist in private practice in Sacramento. It sells four products, only from its own website: two scalp serums, a shampoo and a conditioner.
None of them contains minoxidil or any other drug ingredient, and the brand markets the system as minoxidil-free. The serums are meant to be used together, one dropper (about 1 ml) of each, every day.
The two scalp serums
The Treatment Scalp Serum is built around Redensyl (from larch wood and green tea), with caffeine, saw palmetto, rosemary, ginseng, amla, pumpkin seed, turmeric and pine bark. The Prevention Scalp Serum carries Capixyl (acetyl tetrapeptide-3 with red clover), Procapil (biotinoyl tripeptide-1, apigenin, oleanolic acid), a pea sprout extract, vitamin C and piroctone olamine, an anti-dandruff ingredient.
On paper, that is a thorough list of the cosmetic actives with some research behind them. Both serums contain fragrance, and the Prevention serum contains alcohol.

Does KilgourMD work? What the trial shows
KilgourMD's trial followed 43 women aged 30 to 65 using the full system for 120 days, run by a contract lab in Spain and published in the Journal of Drugs in Dermatology in 2026, with Dr. Kilgour as a co-author. By day 120, hairs shed in a combing test were down 60.3% from baseline, and a dermatologist's rating on the Ludwig scale had improved 11.1%.
Those are real measurements, which is more than most serum brands offer. But the trial was open-label with no control group, so every result is compared only with the start. Shedding naturally rises and falls with the seasons and with stress, and without a placebo arm you cannot tell how much of the drop the serums caused. At day 45, the brand's own results page says the dermatologist saw no statistically significant change.
What the ingredients have behind them
Some of the actives have independent controlled trials: a Redensyl-containing lotion beat a vehicle in a 44-person trial, and rosemary oil was compared with 2% minoxidil in a trial in men. Saw palmetto has a systematic review behind it.
Others rest mainly on supplier or lab data. We found no independent trial of Capixyl on its own, and the brand's pumpkin-seed figure comes from a trial of oral capsules in men, not of anything applied to the scalp. Some figures on KilgourMD's pages are much higher than its own trial results, so judge it by the trial.
The shampoo and conditioner
The Shampoo is sulfate-free with piroctone olamine and panthenol, and KilgourMD says it is gentle enough for daily use. The Conditioner is billed as weightless. Both are fragranced, with a yuzu, bergamot and cedarwood scent.
They were part of the trial routine, but nothing suggests they matter as much as the serums. If budget is the issue, buy the serums and use any gentle shampoo; see the best shampoo for thinning hair.
The price, honestly
The two-serum bundle is $150 for a month, and the brand's FAQ says to allow three to six months. Four months is about $600, or $400 for the three-month supply plus one more month.
There is a 90-day refund guarantee, which helps, but 90 days is shorter than the brand's own timeline for results. Photograph your part in the same light before you start so you can judge it honestly.
Is KilgourMD good for fine hair?
The serums are light, water-based liquids applied to the scalp, so they will not flatten the lengths. The brand says to avoid washing for about 12 hours after applying, so evening use suits most fine-haired readers.
Part the hair in rows and drop the serum onto the scalp rather than the hair. If your roots look greasy the next morning, you are using more than the 1 ml the current directions call for.
Side effects and who should skip it
The trial reported no adverse effects, and on-site reviews are positive. The complaints that do appear are price, a strong scent, droppers that do not fill well, and one report of grey hair turning orange with the Treatment serum. Patch-test both serums behind the ear for two days.
Ask a doctor first if you are pregnant or breastfeeding, and see one rather than buying a serum if your loss is sudden, patchy or comes with a sore scalp. Why is my hair thinning? explains what to rule out, and thinning hair treatments by cause covers the options with stronger evidence.
